Once at the hospital, hand about until you stumble across a hole in the ground. Use a weapon to break the barrier blocking the entrance. Once inside, you will find writings on the wall that will talk about a person that got lost trying to find a way outside.
Follow the path and break every barrier until you make it out of the Hospital. Once out, a cutscene will play. Immediately after the cutscene ends, interact with the item in front called the Severely Eroded Hoe; doing so will give you the Escape from the Hospital achievement in Genshin Impact.
